SPRING SEMESTER: DUE DECEMBER 1ST (SPRING START & NEW TRANSFER STUDENTS ONLY)
These are the steps you need to take to comply with requirements about immunizations and your health history. Information about these steps and the forms themselves are available on the health portal.
Undergraduate students must complete all the steps listed.
Most graduate and Professional and Continuing Education programs do not have immunization requirements but please check with your program for specifics and follow the Steps below if required.
Step 1: Obtain your Official Immunization Record from your Healthcare provider using their form OR the signed ̽̽ Form
Step 2: Upload the completed form and Enter the vaccination dates via
- Log in using your ̽̽ NetID to .
- Click “Medical Clearances” on the left side of the main screen.
- Click “Upload” to add a picture/image/pdf of your vaccination documentation.
- Enter dates for each dose and choose the vaccine name from the drop-down list.
- Be sure to complete the process by clicking “Done.”
